Geny, mal\u00e9 a nezn\u00e1m\u00e9<\/h3>\n

V\u011bdeck\u00e1 definice genu1<\/sup> by zn\u011bla \u201ekr\u00e1tk\u00fd segment DNA tvo\u0159en\u00fd proteiny\u201c. Jin\u00e1 definice \u0159\u00edk\u00e1, \u017ee je to \u201ep\u0159enositeln\u00e1 jednotka biologick\u00e9 d\u011bdi\u010dnosti\u201c. Pokud je tedy gen kr\u00e1tk\u00fdm segmentem DNA, naz\u00fdv\u00e1me genom celkovou sekvenci DNA, kterou vlastn\u00ed \u017eiv\u00e9 bytosti nebo ur\u010dit\u00fd druh. Ale pou\u010dn\u00fdm nebo metaforick\u00fdm zp\u016fsobem lze \u0159\u00edci, \u017ee geny jsou postavami na\u0161eho p\u0159\u00edb\u011bhu.<\/p>\n

DNA je nukleov\u00e1 kyselina, kter\u00e1 obsahuje genetick\u00e9 instrukce pou\u017e\u00edvan\u00e9 p\u0159i v\u00fdvoji a fungov\u00e1n\u00ed \u017eiv\u00fdch organism\u016f.<\/p>\n

Jej\u00ed hlavn\u00ed funkc\u00ed je ukl\u00e1d\u00e1n\u00ed informac\u00ed pro stavbu bun\u011bk, protein\u016f a dal\u0161\u00edch molekul. V ka\u017ed\u00e9 lidsk\u00e9 bu\u0148ce se nach\u00e1z\u00ed v\u00edce ne\u017e 22 000 gen\u016f seskupen\u00fdch do 23 p\u00e1r\u016f chromozom\u016f s nezbytn\u00fdmi informacemi pro n\u00e1\u0161 r\u016fst, v\u00fdvoj a fungov\u00e1n\u00ed2,3<\/sup>.<\/p>\n

DNA je nav\u00edc zodpov\u011bdn\u00e1 za d\u011bdi\u010dn\u00fd p\u0159enos, tedy za genetickou informaci, kter\u00e1 se p\u0159en\u00e1\u0161\u00ed z rodi\u010d\u016f na d\u011bti. Jak se ale tato genetick\u00e1 informace p\u0159en\u00e1\u0161\u00ed z generace na generaci?<\/p>\n

V\u0161ichni m\u00e1me dv\u011b varianty ka\u017ed\u00e9ho z na\u0161ich gen\u016f, jednu d\u011bd\u00edme po matce a jednu po otci. Ka\u017edou takovou verzi genu naz\u00fdv\u00e1me alela. Nap\u0159\u00edklad gen ABO4<\/sup>, kter\u00fd k\u00f3duje protein lidsk\u00e9 krevn\u00ed skupiny ABO, m\u00e1 alespo\u0148 t\u0159i alely: A, B a O.<\/p>\n

Kombinace gen\u016f, kter\u00e9 jsme zd\u011bdili, se naz\u00fdv\u00e1 genotyp, zat\u00edmco fenotyp je v\u00fdsledkem toho, jak se na na\u0161em t\u011ble projev\u00ed, \u017ee je nositelem t\u011bchto gen\u016f v interakci s prost\u0159ed\u00edm.<\/p>\n

Jsou mutace dobr\u00e9 nebo \u0161patn\u00e9?<\/h3>\n

Genom v\u0161ech lid\u00ed na sv\u011bt\u011b je z 99,9 % podobn\u00fd. Jakkoli se n\u00e1\u0161 vn\u011bj\u0161\u00ed vzhled li\u0161\u00ed, li\u0161\u00edme se sotva o 0,1 %5<\/sup>. A pro\u010d vypad\u00e1me tak odli\u0161n\u011b? Proto\u017ee mal\u00fd po\u010det gen\u016f zahrnuje mnoho genetick\u00fdch variant zodpov\u011bdn\u00fdch za fenotypovou variabilitu, kterou vid\u00edme a vn\u00edm\u00e1me.<\/p>\n

Mutace je hlavn\u00edm zdrojem genetick\u00e9 variability6<\/sup>. Je to z\u00e1kladn\u00ed vlastnost DNA, d\u00edky kter\u00e9 prob\u00edh\u00e1 evoluce, ve kter\u00e9 hraje roli p\u0159\u00edrodn\u00ed v\u00fdb\u011br a predispozice. A\u010dkoli je frekvence nebo rychlost mutac\u00ed velmi n\u00edzk\u00e1 (\u0159\u00e1dov\u011b 10-5 na osobu nebo \u017eivou bytost a generaci), opakuj\u00edc\u00ed se mutace genu m\u016f\u017ee v populaci zp\u016fsobit z\u00e1m\u011bnu jedn\u00e9 alely za jinou.<\/p>\n

Mutace m\u00e1 preventivn\u00ed adaptivn\u00ed charakter; to znamen\u00e1, \u017ee se d\u011bje n\u00e1hodn\u011b, ani\u017e bychom v\u011bd\u011bli, jestli bude pro organismus u\u017eite\u010dn\u00e1 nebo \u0161kodliv\u00e1, ale jakmile se to stane, zap\u00ed\u0161e se to do jeho genomu.<\/p>\n

Je Alzheimerova choroba d\u011bdi\u010dn\u00e1?<\/h3>\n

V\u011bt\u0161ina z n\u00e1s zn\u00e1 Alzheimerovu chorobu, nebo u\u017e o n\u00ed sly\u0161ela. Jedn\u00e1 se o typ neurodegenerativn\u00edho onemocn\u011bn\u00ed, kter\u00e9 zp\u016fsobuje p\u0159edev\u0161\u00edm probl\u00e9my s pam\u011bt\u00ed a chov\u00e1n\u00edm. Symptomy se obvykle rozv\u00edjej\u00ed kolem 65. roku v\u011bku a \u010dasem se zhor\u0161uj\u00ed, a\u017e do chv\u00edle, kdy znemo\u017e\u0148uj\u00ed ka\u017edodenn\u00edm \u017eivotn\u00ed \u00fakony a \u010din\u00ed posti\u017eenou osobu zcela z\u00e1vislou na ostatn\u00edch.<\/p>\n

V\u00edme ale skute\u010dn\u011b, co se d\u011bje uvnit\u0159 t\u011bla a jak\u00e9 jsou mo\u017en\u00e9 p\u0159\u00ed\u010diny? V mozku posti\u017een\u00e9m Alzheimerovou chorobou jsou neurony po\u0161kozeny nahromad\u011bn\u00edm protein\u016f (Amyloid a Tau), kter\u00e9 zp\u016fsobuj\u00ed, \u017ee mezi sebou ztr\u00e1cej\u00ed spojen\u00ed7<\/sup> a nakonec vyvolaj\u00ed jejich z\u00e1nik.<\/p>\n

V 95 % p\u0159\u00edpad\u016f je Alzheimerova choroba v\u00fdsledkem kombinace genetick\u00fdch8<\/sup> faktor\u016f, faktor\u016f \u017eivotn\u00edho prost\u0159ed\u00ed a \u017eivotn\u00edho stylu, kter\u00e9 \u010dlov\u011bka v pr\u016fb\u011bhu \u017eivota ovliv\u0148uj\u00ed. Zb\u00fdvaj\u00edc\u00edch 5% p\u0159\u00edpad\u016f, definovan\u00fdch jako \u010dasn\u00e1 nebo d\u011bdi\u010dn\u00e1 Alzheimerova choroba9<\/sup>, se obvykle objev\u00ed p\u0159ed 65. rokem v\u011bku s agresivn\u011bj\u0161\u00edm a\/nebo rychl\u00fdm zhor\u0161ov\u00e1n\u00edm, zejm\u00e9na v d\u016fsledku mutac\u00ed v genech: amyloidn\u00ed prekurzorov\u00fd protein (APP), presenilin -1 (PSEN1) a\/nebo Presinilin-2 (PSEN2).<\/p>\n

Genetick\u00fd pod\u00edl na riziko vzniku Alzheimerovy choroby se odhaduje na 60\u201380 %10<\/sup>\u00a0\u00a0 a pravd\u011bpodobn\u011b se skl\u00e1d\u00e1 z kombinace b\u011b\u017en\u00fdch a vz\u00e1cn\u00fdch alel se v\u00fdrazn\u00fdm m\u00edrn\u00fdm a\u017e st\u0159edn\u00edm vlivem na riziko onemocn\u011bn\u00ed, nav\u00edc k interakc\u00edm gen-gen a gen-prost\u0159ed\u00ed.<\/p>\n

Nyn\u00ed se v\u00a0budete v duchu pt\u00e1t, na jak\u00e9 \u00farovni je sou\u010dasn\u00fd genetick\u00fd v\u00fdzkum? V kontextu sou\u010dasn\u00e9 lidsk\u00e9 genetiky umo\u017enil v\u00fdvoj nov\u00fdch technik navrhnout celogenomov\u00e9 screeningov\u00e9 studie (GWAS) identifikuj\u00edc\u00ed v\u00edce ne\u017e 80 gen\u016f11\u201313<\/sup> spojen\u00fdch s Alzheimerovou chorobou, z nich\u017e nejv\u00fdznamn\u011bj\u0161\u00ed je gen APOE, detekovan\u00fd p\u0159ed t\u00e9m\u011b\u0159 30 lety14,15<\/sup>. Navzdory tomu tyto sign\u00e1ly p\u0159edstavuj\u00ed sotva 31% genetick\u00e9 variace onemocn\u011bn\u00ed, co\u017e znamen\u00e1, \u017ee v\u011bt\u0161ina genetick\u00e9ho rizika z\u016fst\u00e1v\u00e1 st\u00e1le nejasn\u00e116<\/sup>.<\/p>\n

Pozn\u00e1n\u00ed rizika Alzheimerovy choroby za pomoci prediktivn\u00ed medic\u00edny<\/h3>\n

V sou\u010dasn\u00e9 dob\u011b si lze budoucnost zdrav\u00ed lid\u00ed p\u0159edstavit \u010dten\u00edm jejich genomu. Dosud byla medic\u00edna preventivn\u00ed, diagnostick\u00e1 a terapeutick\u00e1, ale s rozvojem genomiky m\u016f\u017ee b\u00fdt i prediktivn\u00ed a personalizovan\u00e1.<\/p>\n

Anal\u00fdza DNA \u010dlov\u011bka m\u016f\u017ee poskytnout \u00fadaje o existenci jedn\u00e9 nebo n\u011bkolika mutac\u00ed v jeho genomu, kter\u00e9 ho s ur\u010ditou pravd\u011bpodobnost\u00ed \u00a0predisponuj\u00ed nebo ho \u010din\u00ed v budoucnu17<\/sup> n\u00e1chyln\u00fdm k rozvoji onemocn\u011bn\u00ed, jako je nap\u0159\u00edklad rakovina. A nejenom doty\u010dn\u00e9ho \u010dlov\u011bka, ale i jeho rodinu.<\/p>\n

Personalizovan\u00e1 a c\u00edlen\u00e1 medic\u00edna by neexistovala bez znalosti na\u0161eho genomu. Na z\u00e1klad\u011b genetick\u00fdch variant, kter\u00fdch je pacient s\u00a0ur\u010dit\u00fdm onemocn\u011bn\u00edm nositelem, dok\u00e1\u017eeme p\u0159edv\u00eddat, jak\u00fd druh l\u00e9\u010dby by mu m\u011bl b\u00fdt poskytnut18<\/sup>. Nen\u00ed vyu\u017eita generick\u00e1 l\u00e9\u010dba, a vyb\u00edraj\u00ed se s velkou p\u0159esnost\u00ed ty nejlep\u0161\u00ed formy l\u00e9\u010dby, vyhovuj\u00edc\u00ed jednotliv\u00fdm genotyp\u016fm.<\/p>\n
